Assess the patient's clinical, functional, and emotional status, documenting progress and reporting significant changes to the care team. Educate patients and caregivers on self-care techniques, medications, diet, and other physician-ordered instructions, while using each interaction as an opportunity for meaningful health counseling. MINIMUM REQUIRED QUALIFICATIONS –Compact or VT & NH unencumbered RN license Graduate of an accredited school of nursingBLS2 years of experience in Home Health settingsLENGTH OF ASSIGNMENT – 16 weeksSHIFT / HOURS PER WEEK – DaysSYSTEMS – EpicSTART DATE – ASAPFacility Location Lebanon is located in idyllic Grafton County, just south of Hanover.
