Job Description & Requirements Specialty: Case Management Discipline: RNStart Date: 10/05/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Care Manager – Job Duties Provides case management, utilization review, and discharge planning for acutely ill hospital patients with complex medical needs. Completes patient assessments, develops discharge plans, identifies barriers to discharge, and coordinates care with physicians, nurses, social workers, patients, and families. Participates in daily care coordination and complex case meetings, arranges post-acute services and community resources, and ensures patients and families understand their transition plan. Uses Epic to document assessments, care plans, and discharge needs while promoting quality, continuity of care, and cost-effective outcomes. Preferred Experience: Hospital-based Case Manager, Care Manager, or Discharge Planner experience in an acute care setting.
